CLAIMS REPORTING

It is vitally important that you contact us as soon as possible when an accident happens. When an accident takes place, you can call, fax or e-mail us. For comprehensive claim reporting instructions please refer to the ChemPlan Claims Kit provided at binding. Under all circumstances, you must follow the claim reporting requirements of your policy including but not limited to, providing us written notice of the claim.
